Completing the Information Packet
Below in bolded text you will find additional information for completing your NHS Information Packet by area of evaluation. Please follow these directions explicitly! The Faculty Council will not consider information packets that are incomplete or formatted improperly! Some of this information is also available on the "Qualities of an NHS Member" page, without the packet instructions.
Signatures of Understanding
This form is required to be submitted with the information packet. Completeness is important and all of your activites for service and leadership should be recognized and certified. Below you will find a link to the 'signatures of understanding' form for your use in the case that the one provided is not enough.

General Formatting Information:
Begin your information packet with a title page that includes the title "ECHS NHS Information Packet", as well as your name, grade, address, telephone, and parent/guardian name (first and last). Include your name and grade level as a header on each subsequent page of your information packet. Each section must be titled and begin on a new page in the following order: Character, Service, Leadership. Complete the enclosed "Signatures of Understanding" page and add it to the end of your information packet. It is imperative that your packet follows these guidelines!
Character:
A student of good character upholds principles of morality and ethics, is cooperative, demonstrates high standards of honesty and reliability, shows courtesy, concern and respect for others, and generally maintains a good and clean lifestyle. The character of eligible students, including discipline and referral records, will be discussed by all ECHS-Charlotte faculty members. The Faculty Council will then use their discretion of good character and a simple majority vote to approve students for membership into ECHS-Charlotte NHS. The character of ECHS-Charlotte NHS members will be reviewed each semester by the Faculty Council for any member receiving a referral of any kind. Referrals relating to academic dishonesty will automatically disqualify students on the basis of character. For the NHS Faculty Council to better consider your character, include a concise statement describing your character, expressing your desire to be inducted into NHS and what this honor would mean to you if you were inducted into NHS.
Service:
This quality is defined through the voluntary contributions made by a student to the school or community, done without compensation and with a positive, courteous, and enthusiastic spirit. Eligible students will describe their service contributions to the school or community as part of their information packet to ECHS-Charlotte NHS. The Faculty Council will then evaluate students for service and use a majority vote to approve applicants for membership. Although no minimum number of hours or service projects is required for approval, exemplary members will have service records that have them on track to exceed the level set by Florida's Bright Futures Academic Scholars Program. Include a statement expressing your spirit of service. Describe specific service activities, including the type of service, duration, and your personal involvement. Include sufficient service experience to allow the NHS Faculty Council to make an informed decision. Whenever possible, include the name, contact information, and signature of a supervisor that can verify each specific service activity.
*Scholarship (GPA) does not need to be included in the packet as it is already a prerequisite for consideration! It may be included, yet may not be significant during Faculty Council review.
Leadership:
Student leaders are those who are resourceful, good problem solvers, promoters of school activities, idea contributors, and persons who exemplify positive attitudes about life. Leadership experiences can be drawn from school or community activities while working with or for others. Holding an officer position in a club is in no way a requirement for leadership, nor is it alone an indication of leadership. The Faculty Council will evaluate leadership with an emphasis on a student's ability to initiate and maintain leadership roles and will approve applicants using their discretion and a majority vote. Include a statement expressing your leadership experience. Explain specifically why each experience qualifies you as a leader at ECHS and/or within the community. Describe in detail as many leadership roles as possible to allow the NHS Faculty Council to make an informed decision. Whenever possible, include the name, contact information, and signature of a supervisor that can verify each specific leadership role.
